Information on this website is highly confidential. User agrees that:

1. the reimbursement information provided above will only be used for the purpose of evaluating potential and/or continuing membership within Wisconsin IPA, Inc. (“WIPA”);
2. all such information will be kept confidential;
3. it shall advise all persons who receive or have access to such information that it is confidential and should not be re-disclosed;
4. it shall exercise at least the same degree of care to safeguard such information received from WIPA as it would use if such information were its own confidential information; and
5. it shall promptly notify WIPA of any circumstances in which it has knowledge of or relating to any unauthorized disclosure, or use of or access to, such information.

User understands, acknowledges and agrees that although Wisconsin IPA, Inc. (“WIPA”) has endeavored to provide access to fee schedule reimbursement information on this website, WIPA is not making any representation or warranty, express or implied, as to the accuracy or completeness of such information. In no event shall WIPA or any of its employees, affiliates, sponsors, agents or representatives be liable for any direct, indirect, punitive, incidental, special or consequential damages or damages for loss of profits, revenue, data or down time, whether based in contract, tort, negligence, strict liability or other law or equitable remedies, arising out of or in any way connected with the reliance upon or use of the fee schedule reimbursement information accessed through this website. User further understands, acknowledges and agrees that only those fee schedule reimbursement amounts referenced and/or described in WIPA’s written agreements with its contracted payers have any legal effect. As used herein, the term “User” includes you and User’s practitioners, officers, employees, members, agents, successors and assigns.
